Report Due Dates

Drug Free Communities Support Program (DFC)

I. Program Reports

  1. Program Progress Reports
  2. The Substance Abuse and Mental Health Services Administration (SAMHSA) and the Office of National Drug Control Policy (ONDCP) use these reports to monitor programmatic progress. Reports are filed online through the Coalition Online Management and Evaluation Tool (COMET).

    Semi-Annual Report Due Dates:
    May 10, 2010
    Nov. 10, 2010

  3. Coalition Classification Tool (CCT)
  4. This survey tool measures your coalition's level of development so that ONDCP and SAMHSA can provide appropriate and targeted assistance. It also helps the national DFC evaluator understand the developmental process of DFC grantees. This web-based, annual survey is administered annually.

    May 10, 2010
    (Only applies to Year 1 Grantees)

  5. Mentor Grant Progress Report
  6. Mentor Progress Report Due Date:
    Dec. 1, 2010

II. Financial Reports

SAMHSA and ONDCP use these reports to monitor financial compliance with grant requirements.

  1. Financial Disbursement Report
  2. The report (Form 272) tracks and monitors the disbursement of DFC grant funds. Reports are filed 45 days after the end of each fiscal quarter online through the Division of Payment Management at http://www.dpm.psc.gov under “grants recipient information”. Failure to file reports by the following due dates may result in a hold being placed on grant funds.

    Due Dates:
    January 30, 2010
        (covers October 01 through December 31, 2009)
    April 30, 2010
        (covers January 01 through March 31, 2010)
    July 30, 2010
        (covers April 01 through June 30, 2010)
    October 30, 2010
        (covers July 01 through September 30, 2010)

  3. Financial Status Report
  4. Financial Status Report (FSR) Standard Form 269 (long form) provides a summary of both federal and non-federal funds expended during the budget year. The form is available from http://www.whitehouse.gov/omb/assets/omb/grants/sf269.pdf (PDF) and when completed sent to:

    Rockville, Maryland 20857 (regular mail) or 20850 (overnight delivery).

    For overnight delivery please include the phone number of your SAMHSA grant specialist. The number can be found on your Notice of Grant Award.

    Due Date:
    December 28, 2010

    Note: New Grantees are required to submit their first report on December 28, 2010

  5. Audit Report
  6. If your organization expends $500,000 or more in a year in Federal awards, you are required to submit a single audit, for that year, in accordance with Office of Management and Budget (OMB) Circular A-133.  Additional information on OMB Circular A-133 and the Federal Audit Clearinghouse, where audit reports are to be submitted, is available through http://www.whitehouse.gov/omb/circulars_default/ and http://harvester.census.gov/sac.
